This agreement is to be filled out by vendors for each event they are working on Am Shalom premises (preferred vendors are required to only fill out this form once per year).

Caterers are required to pay a $250 fee per event ($200 for preferred vendors). The fee can be remitted via check payable to Am Shalom. (Fee current through July 1st, 2024)

Caterer must submit a proposed menu and timeline for use of the kitchen/plan for cooking outside on the premises (via the event planner) to Dana at dponcher@amshalom.com at least one (1) month prior to the event and is responsible for the prohibited items listed above.
