AM SHALOM RELIGIOUS AND HEBREW SCHOOL REGISTRATION (Am Shalom MEMBERS ONLY)
WELCOME to
our online registration for the Am Shalom 2010/2011 school year. We ask
that you fill out EACH of the following forms* and submit them via
e-mail. Here’s a check-list to help you with the process.

Make sure that your Adobe Reader software is
current (version 9). It’s a free download that you get at www.adobe.com.

After your Adobe software is current, please
click on each of these links to complete the following forms:
School Registration Form - fill out once to enroll all the
students in your family. 2 pages.
Student Information Form - this is a form that needs to be
completed for each of your
children being enrolled (for example, if you have three children
being enrolled, you will need to fill it out three times.) 2 pages.
Media Release Form
Volunteer Form - we request that a parent or
parents from each family sign up for a minimum of two volunteer hours
per school year.

Non-Custodial
Parent Form to be completed only if there is a parent who does not live in the child(ren)'s home.
*OPTIONAL –
NEW! Mutual
Friendship Request Form. A copy of this form must be
completed by both students’ parents in order to make a mutual friendship
request according to our new friendship
request policy.
Friendship requests made by any other means will not be
considered or honored.
All of
these are explained in greater detail on the forms themselves. We
believe that the forms are self-explanatory, but if you would like
more detailed information, please call Carolyn at (847) 835-4800, ext

Fill out
the forms. Just type
in your information in the spaces provided. Don’t worry; you can’t
accidentally erase any part of the form, other than if you want to
make corrections to your own information. You will be unable to send
the form until you fill in all of the required fields. The program
will prompt you if some piece of required information is missing. (You may get a message warning
you that only your form data will be saved. That's okay! The data is
all that we need.)

PC users ONLY (Mac Users, please skip ahead to
next step): E-mail each form back. At the
bottom of each form, you will find two “buttons” – one that says “Submit by Email (PC Users ONLY)”, and one
that says “Print in order to fax, file or mail (Mac users
MUST choose this option)”. When you click on “Submit by Email (PC Users ONLY)”, an
e-mail should automatically open up from your e-mail program with your
completed form attached. Just hit send! You can then use the “Print in order to fax, file or mail (Mac users
MUST choose this option)” button if you would like to make a copy for
your records. If an e-mail does NOT automatically open with the form
attached when you click on “Submit by Email”, or if you use a
web-based mail service such as hotmail or gmail, then save a copy of the forms
DATA somewhere on your computer where you can easily find the files (like
your desktop). The files should have the file extension “.xml”.
Then you can create
your own new e-mail message in your regular e-mail
program, attach
the files you created, and send them to carolynf@amshalom.com.
You will
need to follow the above listed procedure for each form that you
submit. Also, please remember that you need to submit a copy of the
Student Information Form for each child being enrolled.

Mac users and any others unable to e-mail your
forms: Please use
the “Print in order to fax, file or mail (Mac users
MUST choose this option)” button to print your forms, then return them to
Am Shalom via regular mail or by fax.
You can also drop them off any time that the Am Shalom offices
are open. We’re sorry, but we
have run into issues where forms being saved and submitted by Mac
users via e-mail are coming through blank. Unless you have Adobe Acrobat Professional installed on your Mac, you will need to print
and mail or fax in your forms. We are sorry for any
inconvenience! Anyone else
having difficulty e-mailing their forms may also use this option.

MAIL IN OR E-MAIL US WITH YOUR PAYMENT. You can
charge your Am Shalom school tuition to your Visa or Mastercard by
e-mailing our Controller, Zoe Sherman, at zoes@amshalom.com, or you
can mail in your check. With
our Early Bird Bonus, classes are $325 per class per child through
June 1st. After June 1st, our regular tuition rates
of $425 per
class per child apply. If you have any questions regarding
your payment options, you can call Zoe at (847) 835-4800, ext 204, between
the hours of 8:00 AM-4:00 PM Monday through Thursday, and 8:00
AM-2:00 PM on Fridays.
NOTES FOR NEW MEMBERS

New members must complete the membership
application process before registering for school.
·
As a one-time courtesy, new members only pay $325
per class per child, regardless of registration date.
Our Am Shalom
school registration is NOT outsourced. The only people who will see
your information are the same people who’ve dealt with it in the past
– members of the Am Shalom staff. We know that you value your
privacy, and so do we.
